Upon acceptance, I do hereby give Bike the UK for MS its assigns, licensees, and legal representatives the irrevocable right to use my name, picture, portrait, or photograph in all forms and media and in all manners, including composite, for advertising, for publication or any other lawful purposes, and I waive any right to inspect or approve the finished product, including written copy, which may be created in connection therewith *

PARTICIPANT RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT In consideration of being allowed to participate in any way in the Bike the UK for MS bicycling event, related events and activities (the "BTUKFMS"), I, the undersigned, acknowledge, appreciate, and agree that: Participation in the BTUKFMS involves significant risks of serious bodily harm, including the potential for severe injury, permanent disability, paralysis and death, and while particular rules, equipment, and personal discipline may reduce such risks, these risks do exist; I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES (defined below) or others, and whether or not covered by my personal health or other type of insurance; and I assume full responsibility for my participation in the BTUKFMS; I willingly agree to comply with the stated and customary terms and conditions for participation; furthermore, if I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such hazard to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, HOLD HARMLESS, AND WAIVE ANY AND ALL RIGHTS TO CLAIMS FOR LOSS OR DAMAGES AGAINST, Bike the UK for MS CIO, and its officers, directors, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, vendors, suppliers and, if applicable, owners and lessors of premises, property and/or equipment used to conduct the BTUKFMS ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, LOSS OR DAMAGE to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS PARTICIPANT RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT. I FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
